Having this information allows us to best serve your child as we are able to reach you, when needed, know when to celebrate your child's birthday, makes us aware of any allergies, and serves as a photo release form so we can capture our GTAG Kids in action during classes and events. You can opt-in to have your child's photos put on the digital sign out front, too!

Please complete this form for EACH child. A copy will be provided to their KIDMIN leaders, small group leader, and kept on file.

Use the space provided to let us know about any allergies that your child could encounter at church. At times, we provide snacks during classes & activities. We want to be able to accomodate for each child. We might ask that you bring safe foods labeled for them.

Glad Tidings Assembly of God (GTAG WEWA) uses photographs, slides, videos, or illustrations of kids/students for many purposes. Such photographs, videos, or other illustrating materials may be used in bulletins or publications produced by the church, in slide presentations, videos, and/or the church website, social media, or in other similar forms of communication.

This portion of the form allows you as a parent or guardian to choose whether your child may be in a video, photograph, or other illustration used by GTAG WEWA.

GTAG KIDS is a vital ministry to our church & in this community. Sharing photos of our actual children enjoying Kids church and classes is a great way to show the health and vitality of our church to those seeking.

At any time, a parent can request that a photo be taken down and we will do that.
